Submit your application. We strongly recommend applying as early as possible.

Wait for DOE Placement Notification. You’ll receive your child’s offer through your MySchools NYC account.

Required Documents
Once your child receives a placement, families must submit:
- DOE Application Form (from MySchools portal)
- Little Scholars NYC Enrollment Form
- Child’s Birth Certificate
- Proof of Address (2 documents required. Ex: utility bill, lease agreement, government mail
- Parent/Guardian Government-Issued ID
- DOE Medical Form (completed by a doctor, including immunization records)
📌 Medical form must be submitted at least one week before the start date.
